This funding seeks to improve health and well-being for children and youth with special health care needs (CYSHCN) and their families by increasing access to patient and family-centered care, improving the process of transitioning youth into the adult health care system, and adopting health care financing models that achieve cost savings by supporting state Title V programs and stakeholders through training and technical assistance.
