Children with medical complexity are those “with substantial family-identified needs, characteristic chronic and severe conditions, functional limitations, and high health care use.” This initiative seeks to develop and implement innovative care and payment models for children with medical complexity by engaging relevant stakeholders to test strategies and build evidence for optimizing high quality, cost-effective, family-centered care.

Award amounts vary. Any public or private entity is eligible, including Indian tribe or tribal organizations, faith-based organizations, and community-based organizations.
